SATELLITE SYSTEM/SERVICE AGREEMENT

CIRCLE TERM:    1   2   3   YEAR/S    (365 - 730 - 1095 CONSECUTIVE DAYS)

 

 

         I THE BUYER HAVE RECEIVED A SPECIAL PROMOTIONAL PRICE ON MY SATELLITE SYSTEM PURCHASE. THIS PRICE IS MADE POSSIBLE BY MY PROMISE TO ADHERE TO THE TERMS THAT FOLLOW.

 

1. I will subscribe to activated service for term of agreement.

 

2. I will not disconnect my service for any reason during term of agreement.

 

3. I will keep my account with service provider in good standing for term of agreement.

 

4. I will not downgrade below minimum required programming service for term of agreement.

 

5. If my equipment fails I will not cancel or suspend my service, I will contact John         Whitford Communications for repair assistance. 805-461-9514

 

6. I understand my service has been activated for me at time of purchase.

 

7. I state I am a first time subscriber to this service provider.

9. These terms apply to all satellite receivers and modems purchased.

I UNDERSTAND IF I DO NOT FOLLOW ALL TERMS ABOVE OR SERVICE IS DISCONNECTED BY SATELLITE PROVIDER FOR LATE OR NON-PAYMENT I WILL PAY JOHN WHITFORD COMMUNICATIONS A DISCONNECT FEE OF $ 799.00 FOR PRIMARY RECEIVER/MODEM AND UP TO $ 299.00 PER ADDITIONAL RECEIVER. I WILL ALSO RETURN ALL EQUIPMENT TO JOHN WHITFORD COMMUNICATIONS. DISCONNECT FEE AND EQUIPMENT RETURN IS DUE UPON DEMAND. I GIVE CONSENT FOR FOR MY CREDIT CARD TO BE CHARGED FOR ANY DISCONNECT FEES OR EQUIPMENT REPLACEMENT WITHOUT NOTICE. IF MY CREDIT CARD IS DECLINED I WILL PAY CASH WITHIN 7 DAYS OF NOTICE. DECLINED CREDIT CARD OR FAILURE TO PAY DISCONNECT FEES AND RETURN EQUIPMENT WILL RESULT IN BEING REPORTED TO CREDIT AGENCIES. LEGAL ACTION WILL FOLLOW. BASIC INSTALLATION INCLUDES THE FOLLOWING LABOR:

1.   TRAVEL TO CUSTOMERS HOME WITHIN 30 MILE RADIUS.

2.   PERFORM SITE SURVEY TO DETERMINE PROPER DISH PLACEMENT.

3.   CONSULT WITH CUSTOMER CONCERNING DISH PLACEMENT,  WIRE/CABLE ROUTING, POINT OF ENTRY AND GENERAL INSTALLATION INFORMATION. ADVISE OF ANY ADDITIONAL CHARGES.

4.   MOUNT DISH ON APPROVED LOCATION OF SINGLE LEVEL DWELLING.

5.   ROUTE CABLE FROM DISH TO ONE TELEVISION OR COMPUTER. (UP TO 75 FEET) CABLES WILL BE ATTACHED ON EXTERIOR WALLS.

6.   CREATE ONE POINT OF ENTRY THRU SINGLE WALL OF WOOD OR    MASONRY. HOLES WILL BE DRILLED INTO THE DWELLING AND SEALED.

7.   CONFIRM STARBAND OPERATION ON JWC LAPTOP COMPUTER.

8.   DOWNLOAD SOFTWARE AND PERFORM SYSTEM TEST.

9.   COMPLETE PROPER PAPERWORK & ACTIVATE SYSTEM.

10. PROVIDE UP TO 20 MINUTES OF CUSTOMER EDUCATION.

7.   ONCE INSTALLATION BEGINS I AGREE TO PAY ALL CHARGES, EVEN IF WORK IS STOPPED BY ME, MY AGENT OR LANDLORD. ANY CHANGES AFTER STARTING WORK WILL RESULT IN ADDITIONAL CHARGES.

8.   PAYMENT IS EXPECTED UPON COMPLETION IN CASH/CHECK/VISA/MC.

9.   MATERIALS ARE NOT INCLUDED. ADDITIONAL CHARGES APPLY.
